IVF or DI for same-sex couples? What the latest HFEA data tells us

Over the past decade, fertility treatment options for female same‑sex couples have expanded. The latest HFEA data shows a striking shift: more same‑sex couples are starting with IVF using donor sperm, and fewer are choosing donor insemination (DI) (HFEA, Trends and Figures 2024). But this trend raises an important question: is IVF really the best first‑line treatment for people who simply need donor sperm?

At NUH Life, we believe many patients are being funnelled into IVF unnecessarily, and that DI remains a safe, effective, lower‑risk, and more affordable option for most same-sex female couples and solo women.

IVF is more expensive, often without clinical need

IVF is an extraordinary technology, but it comes with significant cost – typically up to £7,000 including donor sperm. DI, by contrast, is far more affordable (see our current price list) and often the most appropriate treatment when the only barrier to conception is access to sperm.

For many same‑sex couples, there is no medical indication for IVF. If the treatment recipient is young, healthy and has open fallopian tubes, DI offers an evidence‑based route to pregnancy without the financial burden of IVF.

IVF is more invasive and higher risk

IVF involves hormone stimulation, egg collection under sedation, laboratory fertilisation, and embryo transfer. It is a remarkable process but it is also physically demanding, with risks including ovarian hyperstimulation syndrome (OHSS), procedural complications, and emotional strain. DI, on the other hand, is simpler and minimally invasive. Treatment is still performed in a licensed clinic using screened donor sperm, and cycles can be timed naturally or with mild medication if needed. For many patients, DI provides a gentler, more empowering path to parenthood.

Success rates are strong, but DI is cost-effective

The latest data from 2023-2025 showed that women under 35 had a 25% chance of a live birth after DI at NUH Life.  And while national average IVF live birth rates for the same age group are almost twice as high at 41% (HFEA dashboard, 18-34, own eggs with donor sperm, 2024), the cost of a conventional IVF cycle is typically three to four times higher than a DI cycle. Reciprocal IVF can be even more costly.

So why are couples choosing IVF?

Several factors are driving the shift:

  • Reciprocal IVF is rising, accounting for one in six IVF cycles among female same‑sex couples in 2022. This is a beautiful option for some families but it is not the only way to share parenthood
  • Some clinics promote IVF as a “more successful” option, even when DI is clinically appropriate
  • Increasing commercialisation of IVF clinics
  • Limited NHS funding
  • Misconceptions that IVF is ‘more modern’ or ‘more reliable’

At NUH Life, we believe patients deserve balanced, evidence‑based guidance, not a default push toward IVF.

Why DI at NUH Life is different

Our donor insemination service is built around local, carefully screened sperm donors and a personalised clinical approach. What makes our DI programme exceptional:

  • Local donors recruited through the Nottingham Sperm Bank: rigorously screened, altruistic, and committed.
  • High‑quality donor sperm, tested for motility, morphology, infectious diseases, and genetic conditions.
  • Thoughtful cycle planning, including natural‑cycle DI and mild stimulation options.
  • Continuity of care from a specialist team experienced in supporting same‑sex families.
  • Lower cost, lower risk, lower intervention, without compromising success.
  • Exceptionally high levels of patient satisfaction – our Net Promoter Score(R) is 93 (154 DI and IUI patients surveyed from January 2025 to September 2026).

For many same‑sex couples, DI is not just an alternative to IVF, it is the right first‑line treatment.
